Appointment request form for Email
Receive each appointment request as an email — service, preferred time, and notes laid out.
For a solo practice or small team, an email per request is plenty. This form captures the service, preferred date and time, and any notes, so you can confirm in one reply.
Notifications send from an authenticated domain with Reply-To set to the client, so confirming the time reaches them and clears your spam checks.

How it works
- 1
Deploy the form
Start from our appointment request template — fields, validation, and logic already set up. Customize anything; it's yours.
- 2
Connect Email
Point the Email connector at your inbox — no account linking required.
- 3
Collect on autopilot
Every submission sends a notification to your Email inbox in real time, with your form fields mapped to the right email lines.
How your fields map to Email
Each form field becomes a email line. Edit the mapping anytime in the form editor.
| Form field | Email email line |
|---|---|
| Your name | Your name |
| Phone (optional) | Phone |
| What's this for? | What's this for? |
| Preferred date | Preferred date |
| Preferred time | Preferred time |
| Anything we should know? | Anything we should know? |
The honest catch
The honest catch: notifications send from a verified Happier Forms domain with Reply-To set to the submitter, so replies reach them and the mail passes SPF/DKIM. Spoofing the visitor's address — the classic mistake — is what lands you in spam.
Questions

Free forever. Unlimited submissions. No credit card.
